Vital Records

By Mail

Fill out the request form. The request form is available at the bottom of this page. Choose the form for the type of vital you are requesting. Mail the request form, a photocopy of your valid government issued ID/driver’s license and a Check or Money Order for $20 to Colchester Town Clerk, 127 Norwich Avenue #101, Colchester, CT 06415

In Person

You can fill out the request form prior to coming in or we have them available in the office. We will need a valid government issued ID/driver’s license and payment. You can pay by check, money order, cash, or credit/debit card. If you use a card there is a $2 convenience fee that the vendor charges.

By E-Mail

Send a request e-mail to Gayle Furman and Kayla Fortier. Attach a copy of the front and back of your valid government issued ID/driver’s license and a request form. Please include a phone number where you can be reached. We will contact you for credit/debit information once we confirm we have the record. You may only pay by credit/debit card when requesting a vital record by e-mail. The fee for the certificate is $20 plus a $2 convenience fee that the vendor charges

*Please be aware you will receive your vital certificate by mail, unless you arrive in person. We can only issue a certified copy of a vital record. No e-mail or faxing of a vital certificate can be done in this office.*
